I understand that exercise carries a risk of injury. I confirm that I am, to the best of my knowledge, physically able to take part in gym activities, and that I have disclosed every medical condition, injury, allergy and medication that staff should know about. Where I have a condition that may affect my ability to exercise safely, I agree to obtain my doctor's clearance before training and to provide it on request.

I agree to use the equipment and facilities only as instructed, to follow staff directions and the gym's rules, and to stop and report to staff if I feel unwell during a session. I accept that, except where caused by the gym's negligence, the gym and its staff are not liable for injury, illness or loss of personal property arising from my use of the facilities.

I agree that membership fees are payable in advance for the plan I choose, that memberships are personal and non-transferable, and that the gym may suspend access while fees remain unpaid. Refund, pause and cancellation terms are those published by the gym at the time of joining.

I consent to the gym storing and processing the details on this form, including health information, for the purpose of managing my membership and keeping me safe while I train. This information is visible only to gym staff who need it and is not shared with third parties except as required by law. I may ask to see or correct my details at any time.

I consent to receiving membership-related messages (receipts, renewal reminders and schedule changes) by email or SMS to the contact details I have given.
